A new hip replacement in the UK can cost anywhere between $12,000-$15,000 when done privately. In the U.S., uninsured individuals are quoted $30,000-$50,000 for the same procedure. A new hip replacement in India, performed by a fellowship-trained orthopedic surgeon in a JCI-accredited hospital, can cost anywhere between $5,000-$9,000 all-inclusive. This is why thousands of patients each year from the U.S., UK, Australia, and Middle Eastern countries are traveling to India specifically for this procedure.

In this article we address the real questions that patients are asking. What is the actual cost of a new hip replacement? Which hospitals should I consider for this procedure? How much time will I need for my recovery? And what could potentially go wrong that no one told me about?

How Much Does a New Hip Replacement Cost in India?

The quick and dirty version of this is $5,000-$9,000 U.S. dollars. That cost can depend on several factors including the type of hip replacement implant, the level of hospital, and the city in which the surgery is performed. The cost of the surgery, anesthesia, a five to seven-day hospital stay, and some physiotherapy while you are hospitalized are usually included in the overall cost. The cost of flying, accommodations upon release from the hospital, and any potential complications are usually not.

The Implant is Usually the Largest Variable in Your Overall Bill

The type of hip replacement implant you receive is usually the biggest single variable in your final bill. There are three different levels of implants in India:

  1. Standard cemented implants — ₹1.5–2.5 Lakh (approximately $1800-$3600). These are commonly used in patients who are older or less physically active.
  2. Uncemented (cementless) implants — ₹2.5–4 Lakh (approximately $3000-$4800). These are typically recommended for active patients under the age of 65 and allow the bone to grow into the surface of the implant.
  3. Ceramic or metal-on-ceramic implants — ₹4–6 Lakh (approximately $5,000-$7,200). These implants have a lower wear rate than other types of implants and are therefore often recommended for patients who expect to live longer than twenty years with the implant.

It's worth noting that U.S.-made implants (Zimmer Biomet, Stryker, DePuy) are available in Indian hospitals and are generally more expensive than locally made implants. Be sure to ask your hospital which type of implant they are using, do not simply assume.

What Is Included in the Cost of the Surgery?

The majority of Indian hospitals will quote a "package" price that includes:

  1. Surgeon and anesthesiologist fees
  2. OT (Operating Theater) charges and disposables
  3. Five to Seven Nights in a standard hospital room
  4. Daily physiotherapy during your hospital stay
  5. Basic post-operative medications during your hospital stay
  6. Any necessary pre-operative blood work and X-rays

What is usually NOT included in the cost of the surgery:

  1. An MRI if it is deemed necessary before surgery
  2. Admission to the ICU if you require it
  3. Upgrades to the implant
  4. An extended hospital stay due to complications
  5. A post-discharge physiotherapy program.

While prices for hip replacement surgery are similar across India, there are regional variations. The primary medical tourism destinations in India are Delhi, Mumbai, and Chennai.

Which Hospitals Are Good Options?

There are currently 69 JCI-accredited hospitals in India. NABH (National Accreditation Board for Hospitals) is India's national accreditation board for hospitals. While it is not as well known as JCI, it is still a very rigorous accreditation process. When looking for a hospital to perform hip replacement surgery, look for hospitals that have dedicated orthopedic departments, are doing a high volume of joint replacements (over 500/year), and have an international patient services department.

Hospitals with High-Quality Orthopedic Departments:

Apollo Hospitals (Multiple Locations like Delhi, Chennai, Hyderabad): Apollo is one of the most well-known hospital chains in India. Their orthopedic departments treat thousands of joint replacement patients each year. Both of their hospitals in Chennai and Delhi are JCI-accredited. Apollo also has a full-service international patient department that will assist with visa letters, airport pickups, and interpreter services.

Fortis Healthcare (Delhi, Gurugram, Mumbai): Fortis Gurugram and Fortis Vasant Kunj in Delhi are both well-reputed for orthopedics. They utilize implants from major international brands and have surgeons who have received training at reputable institutions such as the Royal College of Surgeons. Generally speaking, Fortis is good at providing transparent pricing information; they will provide you with a detailed invoice before you pay for anything.

Medanta-The Medicity (Gurugram): Medanta is a large multi-specialty hospital with one of the highest volume joint replacement programs in India. This would be a good option for patients with complex cases (e.g. bilateral replacement, revision surgery, etc.) or those with comorbid conditions that will require coordinated care.

MIOT International (Chennai): Although less well-known internationally, MIOT is highly regarded in India. They have a high volume of orthopedic surgeries, are JCI-accredited, and are often priced 10-15% lower than comparable Delhi and Mumbai hospitals.

Kokilaben Dhirubhai Ambani Hospital (Mumbai): Kokilaben Dhirubhai Ambani Hospital has a solid reputation in Western India. They use a variety of implant brands and have an increasing number of international patients.

Note: While hospital name-brand is important, surgeon experience is even more important. Be certain to ask how many hip replacements your surgeon does each year. Less than fifty per year is a red flag. Over 150 per year is reassuring.

What Exactly Does the Procedure Entail?

Hip replacement surgery involves replacing the damaged ball-and-socket joint with a prosthetic. The damaged femoral head is removed and replaced with a metal or ceramic ball attached to a stem that is inserted into the femur. The acetabulum is then reshaped and fitted with a prosthetic cup.

Surgery typically lasts anywhere from 90 minutes to 3 hours. Spinal or general anesthesia is administered. Most patients are able to walk with assistance within 24 hours, this is mandatory in order to reduce the risk of developing a blood clot and improve the success of your surgery.

Minimally Invasive Approaches to Surgery

Most major medical centers offer minimally invasive options (smaller incision size, less damage to surrounding muscles) for hip replacement surgery. Minimally invasive options typically result in reduced blood loss, quicker initial recovery times, and less postoperative pain. Ask your hospital what type of minimally invasive approach they use, posterior, muscle-sparing (anterior or anterolateral).

What Happens During My Hospital Stay?

During your first few days in the hospital, you will likely undergo the following:

Day 1-2: Surgery, monitoring, pain control, and the first physiotherapy session (you will begin with gentle movements and standing.)

Days 3-5: Stair training, walking with a walker, and physiotherapy twice daily.

Day 6-7: Assessment for discharge. Most hospitals require you to be able to walk independently with a walker before allowing you to be discharged.

Recovery: The Part That Patients Typically Downplay

Recovering from hip replacement surgery is difficult, especially in the first six weeks. Recovering from hip replacement surgery while abroad is even more challenging.

First 6 Weeks Post-Surgery: You Will Need to Remain Near the Hospital

You will need to remain near the hospital until your follow-up appointment. Most hospitals require at least one follow-up appointment before releasing you to fly. Swelling, fatigue, and some pain are typical during the first 6 weeks. Do not attempt to recover from your surgery in a hotel room alone; you will need someone with you.

Traveling Home: Important Travel Information

Most surgeons advise against flying for 4-6 weeks after undergoing hip replacement surgery. Blood clot risk (Deep Vein Thrombosis) increases immediately after surgery and is increased again by long flights. Wear compression stockings and take blood thinners as directed.

Post-Discharge Physiotherapy: Very Important

Indian hospitals will provide you with a discharge physiotherapy plan. You must continue to participate in physiotherapy after you return home, for 3 to 6 months. Arrange for physiotherapy in your home country before you travel. Some patients return home and realize that their GP does not understand how to manage post-op care for patients who have undergone surgery in India. This is something that you should research and arrange ahead of time.

Full Recovery: The Time Frame for Recovery

Patients generally return to normal daily activities within 3 months. Low-impact sports (e.g. swimming, cycling) can be resumed after 6 months. Bone integration of cementless implants requires a full 12 months to complete.

Complications That May Arise

Complication rates for hip replacement surgery in JCI-accredited hospitals in India are comparable to complication rates at Western hospitals. Infection rates are generally less than 1-2%. Deep vein thrombosis (blood clot formation) occurs in less than 1-3% of patients, provided that they are properly treated with blood thinners. However, there are specific risks associated with being a medical tourist. Follow-up gaps: If something goes wrong with your surgery after you return home (a wound infection develops, an implant problem arises, etc.), your local health care providers will not have access to your surgical records. Your surgeon in India will be 12 hours away. Obtain all documentation regarding your surgery — operative reports, implant records (brand, model, lot #), discharge summary, and any diagnostic images. Maintain paper copies of these documents.

Insurance: Coverage for Complications

Most travel insurance policies do not cover elective surgery. Specialized medical tourism insurance policies exist (Tokio Marine, Global Protective Options, etc.). You should purchase specialized medical tourism insurance.

Logistics Surrounding Revision Surgery

If the implant requires revision, it will almost certainly occur in your home country. Ensure that your local orthopedic surgeon has all of your implant documentation before this becomes an emergency.
