Thailand has emerged as one of the best destinations for FIV, with thousands of childless couples heading there for affordable advanced fertility treatment. IVF provides top-quality treatment at a fraction of the cost of the West with state-of-the-art fertility clinics, internationally certified specialists, and high success rates.
With a large number of IVF clinics to choose from, you may be thinking of how to find the best clinic for you. You might be flooded with questions such as what a suitable clinic for IVF is, what factors I should consider, and which IVF clinic has the best reputation?
However, if you have gotten yourself ready, armed yourself with a checklist of what you should be looking for, then you are probably going to have made the right decision. Here is our practical, fundamental guide to help you narrow down your choices when choosing what IVF clinic is best for you.
What is IVF?
IVF involves hormone treatments that increase egg production from one egg a month to several. If tests and ultrasounds determine that eggs are ready for fertilization, egg retrieval will be performed using a hollow needle, choosing only the best eggs. Once retrieved, the eggs will be combined with your partner's best sperm specimen. The specimens will then be kept in a culture dish, where measures will be taken to ensure that the likelihood of fertilization is optimized.
Is IVF for me?
Usually, it is the final frontier in the process of fertility treatments. IVF is performed in cases where:
- Blockage or scarring of the fallopian tubes: A blockage would prevent the embryo from making its way to the uterus.
- There is an ovulation disorder: When ovulation is inconsistent or doesn't happen at all, there will be fewer eggs that can become fertilized.
- Low ovarian reserve: It is a condition whereby women experience a decline in the release of eggs.
- Endometriosis: This is a condition where the tissues of the uterus grow outside of it. It actually damages the normal functionality of ovaries, fallopian tubes and the uterus.
- Fibroids: Uterine fibroids are benign tumors that interfere with implantation of the fertilized egg.
- Impaired Sperm Production: It may be very difficult for the process of fertilization of the egg if your partner has low or no function or production of sperm.
Step by Step Guide to IVF
Initial Phase
Time: 45 - 60 minutes
The visit will start with the evaluation of your uterus and fallopian tubes by ultrasound examination, a hormonal panel, and other necessary testing. This thorough check-up is essential for your doctor to get an overview of your health and decide the most suitable treatment plan for you.
You will also get all the information regarding your treatment cycle, be shown the technique of self-injecting hormones, and be provided with instructions on the usage of other drugs.
Ovarian Stimulation Phase
Time: 10 - 12 days
This stage involves the use of hormonal medication to stimulate the ovaries to produce a maximum number of the follicles. It is done through the self-injection of hormones in the early days of the next menstrual cycle and the process is continued for the following 9-12 days. The more the follicles, the more the mature eggs that can be retrieved, hence higher chances of success.
Before each injection, you will visit the clinic for 12 days. After that, blood tests and ultrasounds are done for 12 days to see the development of your ovaries to confirm the drug therapy.
When the eggs are ready, the physician will give you a “trigger shot” to free the eggs that are matured from the follicles and start the egg retrieval process.
Egg Retrieval and Sperm Collection
Time: 1 - 2 hours
The eggs will be taken out of your follicles by suction through a minor surgical operation 36 hours after the trigger shot. To make the operation relaxing and to ensure that it is painless, general anesthesia will be given. It is usually quickly done between 15 and 20 minutes.
For semen collection, masturbation by the male is advised in order to prevent the mixture of different body fluids from the male and the female.
The ejaculated sperm is put under a microscope to check its quality in a process called semen analysis. This analysis is extremely important as it will suggest the suitable fertilization techniques to use if the physician decides standard fertilization IVF is applicable.
The eggs and sperm are cultured in a laboratory dish and left to undergo natural fertilization.
Embryo Culture
Time: 5 - 6 days
Once the sperm and eggs have been united, the resulting embryos will be cultured for 5-6 days to the blastocyst stage in the laboratory. Then, the embryos at this stage will be able to be transferred and get implanted in the lining of the uterus. An embryo that does not develop until the blastocyst stage is, essentially, considered too weak and incapable, thus it will not be transferred to the uterine cavity.
Embryo culture is a very delicate process and demands the skill and experience of embryologists, who not only need to be experts in regulating environmental parameters, but also adept at operating advanced technical equipment, in order to keep the laboratory a stable environment for the embryos to develop.
Embryo Transfer
Time: 2 - 4 hours
Finally, the chosen blastocyst(s) will be moved to your uterus via a straightforward and painless procedure where the blastocyst(s) come in a fragile catheter, which is going to be put through the cervix into your uterus under the guidance of an ultrasound, and the blastocyst will be delivered into your uterine cavity. The blastocyst(s) will connect themselves to the uterine lining (endometrium) and thus, their development will begin.
Pregnancy Test
One week to ten days after the embryo transfer, you can have a blood pregnancy test to confirm whether you are pregnant. An ultrasound can be done for confirmation about two weeks later.
